Graduate Students Health Requirements
Graduate Students are not required to have a physical examination on file. You cannot register, however, until you have completed and submitted the Medical History / Immunization requirements via the Patient Portal.
North Carolina state law (General Statute 130A-155.1) requires anyone entering college to present a certificate of immunization that documents their compliance with all required immunizations.

Students registered only in off-campus courses:
- Distance Learners: The N.C. Immunization Law does not mandate you to complete any forms. Please contact the Medical Records department at (336) 334-7880 about your intent to enroll. Please note that at any time you are considered an on-campus student, you will have to meet N.C. Immunization requirements.
Students only attending classes scheduled at 5:00 pm or later:
- Post-Baccalaureate Studies (PBS): Students who take 5 or more hours on campus, before 5:00 p.m. are required to meet the N.C. Immunization requirements. Please download the Medical Health Packet form. See the Patient Portal.
- Students enrolled only in weekend classes.
Students taking a course load of four (4) credit hours or less per semester:
- Teacher Certification / Certificate: Students who are considered visitors taking 3 hours or less, attending the university to complete certificate courses, must individually contact the Medical Records office for additional advisement.
