Immunizations and Health History Policy
North Carolina law requires all entering students to submit proof of required immunizations to maintain enrollment at North Carolina A&T University. If you have not been immunized but are immune-based on blood tests or having had the diseases, you must submit official documentation from your medical provider (personal physician, pediatrician or county health department) that you had the respective disease(s) or blood test results.
North Carolina Immunizations requirements vary depending on student classification and/or age. The following instructions will assist you in completing the correct North Carolina Immunization requirements applicable to you:
Student Classification
Distance Learners:
The North Carolina Immunization Law does not mandate you to complete any forms. Please contact the Medical Records department at (336) 334-7880 about your intent to enroll. Please note that at any time you are considered an on-campus student, you will have to meet North Carolina Immunization requirements.
Post Baccalaureate Studies (PBS):
Students who take five or more hours on campus, before 5 p.m. are required to meet the North Carolina Immunization requirements.
Teacher Certification / Certificate:
Students who are considered visitors taking three hours or less, attending N.C. A&T to complete certificate courses, must individually contact the Medical Records office for additional advisement.
International Students:
Vaccines are required according to age (refer to incoming students). International students are required to have a Tuberculin (TB) test within one year of the start of their first semester. A chest x-ray is required if the test is positive.
Undergraduate Students:
North Carolina law requires all entering students to submit proof of required immunizations to maintain enrollment at the University. You are required to complete the Medical Health Packet via the Medicat Patient Portal and submit immunization records to the Medical Records Department.
Graduate / Doctoral Students:
- If you are taking five hours or more on campus, before 5:00 p.m. you cannot register until you have completed and submitted the Medical Health Forms via th Medicat Patient Portal.
- Students 50 years of age and older taking five hours or more on campus have to present proof of the three tetanus series with one dose lasting through your matriculation at the University. You are also required to complete the medical health required forms via the Medicat Patient Portal.
Information for all incoming students:
The following age requirements will assist you in completing the correct North Carolina Immunization requirements applicable to you:
Students 17 years of age and younger:
- 3 DTP (Diphtheria-Tetanus-Pertussis) or TD (Tetanus-Diphtheria) doses.
- 1 Td/Tdap (Tetanus-Diphtheria and/or Tetanus-Diphtheria-Pertussis) dose must be within the last 10 years.
- 3 Polio doses.
- 3 Hepatitits B doses
- 2* MEASLES (Rubeola) one dose on or after age 12 months and before age 16 months and a second dose before enrolling in school for the first time. (2MMR doses meet this requirement.)
- 1** RUBELLA (German measles) dose. (after first birthday).
- 2** MUMPS doses (both after first birthday).
- 1 Varicella dose
- 1 dose Meningococcal
Students Born In 1957 Or Later And 18 Years Of Age Or Older:
- 3 DTP (Diphtheria-Tetanus-Pertussis) or TD (Tetanus-Diphtheria) doses
- 3 Hepatitits B doses.
- 1 Td /Tdap (Tetanus-Diphtheria and/or Tetanus-Diphtheria-Pertussis) dose must be within the last 10 years.
- 2* MEASLES (Rubeola) one dose on or after 12 months of age, the second before age 16 months of age and second dose before enrolling in school for the first time. (2 MMR doses meet this requirement.)

International students are required to provide proof of a negative PPD/Tuberculin skin test within 12 months proceeding the first day of classes. If your PPD/TB is positive a chest x-ray may be performed within the United States.
About Exemptions:
Immunization requirements may be waived for students who have certain medical conditions or for religious reasons. Qualifying medical conditions may include pregnancy or breast feeding, diseases which lower resistance to infection, or present use of medications which lower resistance to infection. Religious exemptions include having a system of belief, practices or ethical values which prohibit the use of immunizations. Please be aware that waived students may be removed from classes if an outbreak of the disease occurs. All students requesting immunization exemption are required to come to the Alvin V. Blount Jr. Student Health Center in person within two weeks of their first registered term in order to sign a waiver form and receive verbal and written information about the waiver. To learn more about waivers, call the Medical Records Department at (336) 334-7880 from 8:00 a.m. and 5 p.m, Monday-Friday.
Exemptions include the following:
- Students registered only for off campus courses
- Students only attending classes scheduled at 5 p.m. or later
- Students enrolled in weekend classes
- Students taking a course load of Four (4) credit DAY hours or less per semester.
If a student lives in ON-CAMPUS Housing, NONE of the exemptions apply.
If at any time a student previously exempted changes to on-campus courses, a course load of more than four credit day hours, or becomes an on-campus resident, required immunizations must be submitted at that time.
