Medical History and Immunizations

All international students must provide documented proof of required immunizations. Students who fail to comply with this requirement will not be allowed to register for classes. The Health Center will place a "registration hold" on your student account. The hold will not be lifted until all medical records are turned in and immunizations are completed.

For detailed information on the immunization requirements go to the Student Health Center webpage. There you will also be able to download the medical form. The form should be completed as instructed and returned to the Health Center.
